Mandatory Forms

All students and parents must complete the following forms:

Medical Release Form
Parent Permission for School Sponsored Trip Form
Handbook Consent Form
Parent/Student UIL Marching Band Acknowledgement Form

All the forms must be filled out completely, with the necessary signatures (the Medical Release Form must be notarized), and returned to the band hall by August 24, 2007. No one will be allowed to travel or perform with the band without these forms on file.
